PARENT INFORMATION FORM
The Parent Information Form (PIF) is a comprehensive caregiver survey designed to capture essential information about a child’s early learning experiences, readiness skills, and family demographic, along with access to community resources and supports. Offered for a flat fee of $750 per fiscal year (unlimited children), the PIF provides communities with confidential data that helps identify strengths, gaps, and opportunities to improve school readiness — guiding funding decisions, curriculum enhancements, and policy planning.
