PARENT INFORMATION FORM

The Parent Information Form (PIF) is a comprehensive caregiver survey designed to capture essential information about a child’s early learning experiences, readiness skills, and family demographic, along with access to community resources and supports. Offered for a flat fee of $750 per fiscal year (unlimited children), the PIF provides communities with confidential data that helps identify strengths, gaps, and opportunities to improve school readiness — guiding funding decisions, curriculum enhancements, and policy planning.

Frequently asked questions

ASR’s school readiness assessment model gathers a holistic 360-degree view of children entering school:

Kindergarten/Pre-Kindergarten Observation Form (KOF/P-KOF): A 22-item assessment completed by teachers using both observation and direct assessment. This evidence-based assessment measures early academic skills, self-regulation, social emotional, and motor skills. The assessment data can be entered into an online database where users can immediately access reports showing readiness levels for individual children, classes, schools, and districts.

Parent Information Form (PIF): A caregiver survey designed to gather information on how family structure, early childhood experiences, and available community resources impact school readiness.

Teachers are provided with a 60–90-minute online training along with all assessment materials.

At the training, teachers learn about the purpose of the KOF/P-KOF and how to administer each assessment item. They practice using the Scoring Guide and test scenarios to ensure consistent administration of key items. The session emphasizes objective rating, proper use of accommodations, and understanding how readiness data supports instruction.

Kindergarten Assessments (KOF)

Kindergarten teachers complete the KOF assessments within two to six weeks after the start of the school year. Conducting assessments early ensures results reflect a child’s readiness before formal instruction influences their development.

Preschool Assessments (P-KOF)

Preschool teachers may administer the P-KOF multiple times throughout the school year to monitor progress toward kindergarten readiness over time. This flexible approach supports progress monitoring and data-informed planning throughout the school year.

KOF and P-KOF annual pricing is determined by the number of children who will be assessed for the school year. There is a base price of $300 for up to 200 children and $1.50 per additional child.

The Parent Information Form (PIF) costs a flat fee of $750, regardless of the number of parent respondents.
